House
Bishop House, located at 39 W128th Street, is licensed
by NYS as a Congregate Treatment program that provides on-site restorative
services 24 hours a day for 20 adults in attractive, spacious one-bedroom
apartments. A community room and extensive paved backyard provide
space for recreational and social activities. Bishop House was the
recipient of the last award for Excellence in Supportive Residential
Services conferred by the New York State Office of Mental Health.
Weston
House
Weston House, located at 203 W113th Street, is licensed
by NYS as an Apartment Treatment program that provides restorative
services on-site 24 hours a day for 48 adults with serious and persistent
mental illness. The seven-story elevator building was originally planned
as luxury condominiums with twenty four (24) spacious one and two
bedroom apartments, and includes a community room, and laundry room.
Supported
Housing
Weston United Community Renewal Supported Housing is
a Level I Scattered-Site housing program funded by OMH and DMH to
assist 81 persistently mentally ill adults maintain residence in the
Harlem community. Our goal is to supplement consumer special needs
for the most successful independent living possible. Consumers live
independently either alone or in shared apartments with no more than
two other consumers. Case Managers contact them once a week either
via phone, in groups or individually in person. Consumers prepare
their own meals, do their own laundry and pay their own utilities.
Consumers are not restricted to a curfew. Case Managers are on call
24 hours a day for support.
Casa Renacer Casa Renacer is Weston United Community Renewal's first
permanent
supported housing program.
Casa Renacer offers housing to 60 mentally ill, formerly homeless
men
and women. The program provides an array of on-site services that
include individual and group counseling, substance abuse counseling,
case management, and medication management and monitoring. The residence
is secured 24 hours a day with staff that consists of Social Workers,
Case Managers, Safety and Support Aides, a Psychiatrist, a Nurse,
and a
Residence Manager.
Casa Renacer provides these men and women with
a safe environment to
live in, and empowers them to participate and function in their
community, which improves their quality of life. Eligible tenants
are
required to pay only one-third of their income towards rent.
Community
Focus
The Community Focus Program serves individuals and families
to find and maintain housing in the community of their choice. Located
at 321 West 125th Street, the program is staffed by a Program Director,
Program Coordinator, Case Manager, Life Skills Counselor, Housing
Developer and Driver. Funded through the Human Resources Administration's
(HRA) Supported Housing Assistance Program, Community Focus is part
of the new Scatter Site II (SSII) housing initiative by HRA which
enables community based organizations to locate apartments for families
and individuals in need of housing. Our Scatter Site II model is a
six-to-nine month program provides help in 1) Locating apartments
2) Case Management, and 3) Independent Living Life Skills Training.
Consumers lease their own apartments directly from Landlords. The
primary aim is to ensure that Consumers and their families receive
the support needed to successfully maintain independent living. Eligibility
requirements include: a referral from HRA Supported Housing Assistance
Program, a demonstrated need for housing, ability to live independently,
and willingness to participate.
Weston
Transitional Living Community
Weston United Transitional Living Community (TLC)
is a forty bed homeless shelter for mentally ill men and women. The
TLC is the only freestanding service site within the New York City
shelter system. Funding for the six to nine month treatment program
is provided by New York City Department of Homeless Services and Department
of Health and Mental Health. The programs' primary goal is
to stabilize the individual socially, psychiatrically, economically
and provide access to appropriate supported hosing options. An interdisciplinary
team includes MSWs, a MICA Specialist, a nurse, case managers and
a part-time psychiatrist, with on-site staff 24/7. All TLC referrals
are received from the New York City Department of Homeless Services.
On average 80-120 consumers are served annually.
