Harlem Cultural Connection
Harlem Cultural Connection serves SPMI consumers by providing a supportive
work environment. Consumers support each other by working together
and attending weekly meetings. The goal is to provide rehabilitative
and support activities consistent with the principle that individuals
share a common condition or experience can be of substantial assistance
to each other.
Career Development Program
CDP, funded by NYS OMH, under the direction of a job developer, offers
assistance in job finding, resume writing, interview techniques, job
coaching and placement. The goal of this program is to assist individuals
in maximizing their employability and to provide a service of excellence
to underserved mentally ill individuals in a culturally diverse environment.
CDP was designed for individuals with a mental illness to re-integrate
back into the competitive workforce. Consumers achieve self-confidence,
dignity, respect and a sense of responsibility that comes from work.
Project United
Project United Peer Advocacy is a group of men
and women who work collectively to address the needs of the mental
health community. Project United is made up of people who have faced
the challenges of mental illness, and have overcome their disability
through the methods of self help, medication, group therapy, education
and training. As the arm of the consumer movement at Weston United,
we are dedicated to providing information for consumers of mental
health, training, workshops and support. Our motto is "Turning
disabilities into abilities".

Position: Part Time Licensed Practical Nurse
Education: Medical Degree

Award winning community-based mental health agency located in Harlem, is seeking a part-time (15 hours per week) LPN to be a part of the ACT Team program. Candidate will be responsible for assessing the health needs of consumers and making the appropriate referrals to community based medical care. Provides prescribed medical treatment and personal care to consumers in a residential setting for homeless, mentally ill and chemical abuser. Ensures that consumer's needs are met in accordance with instruction of physician, agency administrative procedures and the ACT Team. Must have a Medical degree. New York State registration and certification as a Licensed Professional Nurse (LPN). Knowledge of State and City regulations with regards to the homeless and MICA population. Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence. Program: Bronx NY/NY III Transitional Supportive Scattered-Site Housing Program
Position: CASAC Case Managers
Education: Bachelor Degree

Job Description
Weston United is seeking two full time CASAC Case Managers to participate in the intake, orientation and screening process. Document program notes as per services provided. Assess the needs and strengths of consumers to set service plan goals and objectives. Write the individual service plans and bi-annual review. Make established minimum number of visits to each consumer on and assigned caseload or as directed by the Program Manager. Will be on call 24 hours for emergency services that may include responding in person.
Qualifications

Prefer a Bachelor degree and two (2) years of related work experience and/or High School diploma or General Education degree (GED) and equivalent combination of work experience.

Must have a CASAC License and experience in mental health and substance abuse assessment and treatment.

Program: Bronx NY/NY III Transitional Supportive Scattered-Site Housing Program
Position: Housing Specialist

Job Description
Weston United is seeking a full time Housing Specialist. Candidate will be responsible for identifying permanent housing for homeless mentally ill and/or substance abuse history adult population. Maintaining case records and perms related administrative work. Responsible for training staff on the housing procedures and application process. Complete and submits all applications. Collaborate with interdisciplinary team to ensure that clients obtain needed supportive services leading to successful placement. Conduct housing and psychosocial groups for consumers. Act as liaison between agencies, clients and prospective housing providers. Review staff follow-up documentation of discharged consumers for at least three months.
Qualifications

Must possess a BA/BS degree in social work or related field.

Minimum of three (3) years experience working with the NYC Housing market with consumers on issues of homelessness, entitlements, addiction, mental illness and special need housing.

Familiarity with all NYC housing application procedures including processing housing applications such as NY/NY, NYCHA, DHS/HSP and Section 8.

Knowledge of CUCS-RPM Vacancy and Information required.

Strong computer literacy, writing, communication, organizational and interpersonal skills necessary.

Program: Bronx NY/NY III Transitional Supportive Scattered-Site Housing Program
Position: Program Manager

Job Description
Weston United is seeking a full time Program Manager who will be supervising two Case Managers, one Housing Specialist and the Peer Custodian. Review and sign off on time and attendance records for staff. Review all charts and conduct monthly chart reviews. Review and monitor staff performance and provide verbal and written feedback including staff evaluations and disciplinary actions when indicated. Provide weekly supervision to staff. Complete or approve all service update forms for admissions, hospitalization and discharges.
Qualifications

Must possess a Masters degree in Social Work and a minimum of two (2) years experience working with severe and persistent mentally ill adults with a history of substance abuse.

Licensed Masters Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW).

Must have good interpersonal, computer, excellent communication and writing skills with the ability to document in accordance with program standards.
